BANIA Spring Event Presents: The Holy Grail of Care Coordination between Paramedics and Hospital

When an emergency happens, it is usually scary, chaotic, and dangerous. Luckily, we have a network of Emergency Medical Service (EMS) systems with Emergency Medical Technicians and Paramedics as the first to respond when time is the essence!  These heroes see humanity at its most vulnerable and precarious moment while they are racing with time to save lives!  In those bleak moments, victims or their families may be unable to provide reliable patient information that can affect initial care decisions and long-term outcomes.  Knowledge of the patient’s relevant health data; such as recent hospitalizations, past medical history, medications, allergies, preferred health care facilities, as well as end-of-life decisions, enables EMS providers to provide the most appropriate pre-arrival care and ensures the patient is transported to the appropriate health care facility.

This Spring BANIA, is honored to introduce a few experts who are intimate to the successful implementation of the SEARCH, ALERT, FILE and RECONCILE (SAFR) grant with Emergency Medical Services Authority (EMSA), San Mateo County Connected Care (SMCCC), and Stanford Health Care (SHC).  This EMSA-SAFR project significantly improves quality and coordination of care between EMS and SHC ED for patients by including key clinical data, ePOLST, CURES, and medication data .  This crucial information was not previously accessible by the EMS providers and only verbal observation was given to the hospital’s Emergency Department.
